注意力变量检查（TOVA）在中国的初步应用

摘要　目的：初步测试TOVA在中国注意缺陷多动障碍（ADHD）患儿中应用的适用性。方法：应用TOVA视觉刺激软件对一组符合DSM－Ⅲ－R诊断标准的ADHD患儿和正常儿童进行检查，并于药物治疗前后分别测试患儿的操作水平，与Conners父母量表分的变化做相关分析。结果：发现ADHD患儿与正常儿童之间TOVA各项变量均存在显著性差异，与临床诊断标准相比，诊断的敏感度85.71％，特异度87.5％；药物治疗前后患儿TOVA的操作水平变化明显，与Conners父母量表分的变化有一定相关。结论：TOVA检查确实能反映ADHD儿童的病理特点，对疗效判定也比较敏感，与Conners儿童行为评定相比，能更全面地反映ADHD患儿治疗前后的变化，作为一种具有跨文化性质和相对客观的检查手段，它在中国有很好的适用性，有一定的科研和临床应用价值。

摘　要：[目的]探讨注意力变量测试(Test of Variables of Attention,TOVA)在儿童注意缺陷多动障碍(attention deficit hyperactivity disorder,ADHD)辅助诊断及鉴别诊断中的意义.[方法]根据DSM-Ⅳ诊断标准,将儿童心理门诊以"注意力不集中、多动"为主诉就诊,且瑞文渐进模型测验智商(intelligence quotient,IQ)≥85分的56例儿童分为ADHD组(34例)和非ADHD组(22例)；分别予注意力变量测试,分析各组测试结果及各参数的辅助诊断意义.[结果]34例ADHD患儿TOVA异常占85.29%；22例非ADHD儿童TOVA异常占27.27%,其中提示ADHD可能的占13.64%,均为行为问题儿童.各参数中,ADHD组遗漏(omission errors,OE)、反应时间(responsetime,RT)、反应时间变量(responsetimevariability,RV)得分及ADHD评分均显著低于非ADHD组(P＜0.01).ADHD组第二部分反应时间(response time in hlf 2,RT2)比第一部分反应时间(response time in hlf 1,RT1)得分显著提高(P＜0.05).[结论]注意力变量测试(TOVA)是客观、标准、高精确度的持续性操作测试,在ADHD诊断中有较理想的辅助诊断意义,值得推广应用.分类号：R749.93　文献标识码：A
